Because your comfort and happiness are important to us, our friendly and supportive staff is available to meet with you at your convenience to talk about becoming part of the Sunapee Cove community and to assist you in determining what services you may need or desire while you are living here.
Residents at Sunapee Cove are able live independently yet have access to support and health services should they need them. Some of our apartments are specifically designed to be handicapped accessible.
Since we strive to give you the most personalized health care possible, we would greatly appreciate if your primary care physician could provide us with health-related information that would assist us. Please help us help you in the best possible manner by providing the following information:
- Medical history
- Recent physical examination report
- Immunization report
- Tuberculin test record
- Current health problem list (if any)
- Current list of prescribed medications, treatments
and any recommended health screenings and/or services
And because providing personal care and attention for each and every resident is our top priority, our resident services team will meet with you every six months to review your service plan.
